Information, Referral & Assistance – Incoming call or visit to the Center by a person (or caregiver of a person age 60 or older) who is seeking information or assistance which may link them to a service or activity provided by the Center, or any other outside agency referred by the Center, that they were not fully aware of before initial contact.
Outreach Outgoing call or visit (one-on-one) initiated by Center Staff or volunteer to persons who may be in need of services provided by the Center or other agencies preferred by the Center. Priority should be given to low-income, socially isolated, older adults who are in most need of services and socialization. Health Screening Activities that utilize diagnostic tools to test groups of persons for the presence or certain risk factors of a particular disease or health condition. Health screening activities include but are not limited to: blood pressure, blood sugar, cholesterol, vision, hearing, foot clinics, etc
Friendly Visitation – Regularly scheduled visits made at least once a week to the residence of elderly persons who are frail, homebound and/or socially isolated, and who live alone (nursing home residents age 60+ also qualify for this service), assistance with activities, providing, reading materials or discussing current events.
Eligibility Due to South Central Tennessee Development District’s Aging and Disability guidelines seniors must be age 60 plus be able to care for themselves and above before I can count their attendance to meet federal requirements, but the center’s board has chosen to encourage anyone age 50 and older to participate in any activity provided at the center. Only seniors age 60 will be counted for our federal reports. |
Health Promotion & Physical Fitness Aerobics, tai chi, yoga or any other classes that involve physical fitness activities that include performing at least 3 measurable outcomes for the “Healthy Seniors” Report in order to receive additional Title III-F Health Promotion funding.
Education Activities to acquire knowledge, experience, skills, or to improve health and well-being. Examples of educational activities may include speakers on health issues, Medicare, crime/accident prevention, history, financial management, etc., and classes on computers, ceramics, quilting, arts, and crafts, etc. Activities may be done through lecture, group. discussion, group audio/video use, demonstrations, and/or supervised hands-on experience.
Recreation – Must be structured activities organized and/or led by Center staff or qualified volunteers. (If participant does an unscheduled activity on their own that is not led or organized by staff/volunteer, it is not structured recreation.) Examples of structured recreational activities include bingo, card and billiard tournaments, shuffleboard and other sports, trips, parties, arts, and crafts, etc.
Telephone Reassurance Prearranged, scheduled telephone calls made at least three times per week (preferably daily) to or form frail, homebound, and socially isolated, elderly person who live alone, to make sure that those individuals are doing well and have not fallen down or fallen ill. This service is to provide a regular check on vulnerable elderly persons who do not have anyone else to check on him or her daily.
